Using this form the End User can submit to its Focal Point a request a new product. In the form there is a classical text part and it is also possible to enter the geographic area using different shapes in the section dedicated to highlighting the area.

The input text boxes to be filled in to submit an order are presented in the following table. Fields marked with an asterisk (*) are automatically filled in with information provided by the User using the User setting application, if available.

Name/Required/* The name of the User that is submitting the request

Surname/Required/* The surname of the User that is submitting the request

E-mail/Required/* The e-mail address of the User that is submitting the request

Alternative e-mail/Optional/* An alternative e-mail address of the User that is submitting the request (e.g. a private e-mail account)

Organisation/Optional/* The Organisation of the User that is submitting the request

Phone (Office)/Optional/* The office telephone number of the User that is submitting the request

Mobile Phone/Optional/* The mobile phone number of the User that is submitting the request

Fax/Optional/* The fax number of the User that is submitting the request

Type of disaster/Required Only for Emergency mode. The type of the disaster(s) for which a product is required. More than one disaster can be selected

Is there a time constraint ?/Optional Using this field it is possible to specify a deadline for the availability of the requested product. This information will be made available to the Focal Point Emergency mode

Description/Required A description of the rationale of this request submission.The information inserted by the User will help the Focal Point and the GMES ERS in providing valuable products.

Area of Interest/Required The coordinates of the area for which the product is requested. The User has the possibility to define one or more than one areas using a combination of these inputs: Predefined areas

• A square
• A circle
• A free polygon (double click to stop editing)
• A circle specifying a point and a radius
• A rectangle specifying the coordinates of the upper left and lower right points.Coordinates can be in decimal-degrees, degrees-minutes-seconds or fixed-width format. E.g: -3.263722, 3° 15' 49.4" W or 0031549.4W

Area name/Required The name assigned to the Area of Interest defined

Product/Map type/Optional The product or map type requested. Please refer to the GMES ERS Starter Package for a complete reference of their content. Allowed values are:

Once finished the form, the request is sent to the Focal Point with the “Submit” button.

An identifier is generated and will uniquely identify the request.
